Putting People First

The Burlington Family Health Team (BFHT) is a healthcare organization that is focused on serving the community.  We offer programs in physiotherapy, psychotherapy, and remote care management , as well as a number of educational sessions on brain health, bone health, and mental health. We are proud to be a member of the Burlington Ontario Health Team.

“Family Health Teams allow Physicians and other allied health providers to work as a team to focus on keeping patients healthy. The vision calls for Physicians, Nurse Practitioners and other members of the team to practice together in a positive working environment, sharing and benefiting from the complementary knowledge and skills of their colleagues.”

Ministry of Health and Long-Term Care

Family Health Teams ensure that people receive the care they need in their communities, as each team is set up based on local health and community needs. They focus on chronic disease management, disease prevention and health promotion, and work with other health care organizations, such as public health units and Community Care Access Centres.
